Hi all,
It has been a while since my last post. We did do IEP testing, and my ds now has an IEP in place for 3rd grade. The first problem to crop up this year has been my son forgetting to bring some items home from school.
The school provides and assignment book, to write down the daily assignments. This must be brought home and signed every night, but a few times a week he doesn't bring it home.
Also, his homework must come home. Now usually he brings home the work and forgets the assignment book for me to see and sign, but sometimes the homework is forgotten.
In his IEP I had them place that he needs help with organizational skills, and that a buddy system may help him. Last year, the teacher basically packed his bag for him. I would like for him to pack it, but for his teacher or a buddy to physically make sure he packed it correctly. I have sent notes, and the teacher said she would start a buddy for him and that she would do a reward chart for the days when he brought everything home and back.
So far, the buddy and the teacher are not reminding him or checking him. He says the teacher just tells him to pack his bag. She is not punishing him so far, usually other kids without IEP would lose some recess time for forgetting the assignment book. So I was thinking about a self-checklist for his desk to look and and physically check off: assignment book, spelling work, reading book, etc, but I can not find one anywhere online.
I was wondering if anyone had a pre-formatted checklist that would work for him? Thanks a bunch!!!!

We have the same exact problem. I actually had it put in his IEP that he is to have a checklist for the packing of the backback at the end of the day. His teacher last year made him up one and it worked for a while but then she "forgot" about it. So far this year he has forgotten something every other day. I wrote a note in his planner to to the teacher reminding her but so far nothing. I don't have a formatted checklist but Im just going to make one up myself and laminate it.
