When I have to fill out those papers from school and it asks about medical conditions do I put Sean's DX(s) on there?

I do on Shannon's, along with her allergies....I guess its up to you.

KK...I also put all the information down on Josh's medical forms. Between the add dx and all his food allergies, I want to make sure if there is a medical emergency that all the proper information is there. I also list all the medications he takes.

I did, including their allergies. I figured, she has an IEP, it's not like it's going to remain a secret for long....and she's not on her meds for a week, so they're going to know today that something is up!!

Thanks. I just wasn't sure. I knew it was important to list his meds so once they read he was on Depakote, Adderall and Hydroxyzine they'd figure something had to be going on.
